Authorization for Payment
    This form is returned with your annual registration forms.  It indicates your method of  tuition payment for the
    upcoming school year and provides the necessary information  for electronic payment of tuition from your
    checking or savings account.  

Comprehensive Child Consent and Release Form
    This form is completed once per school year, providing information and consent relative to school field trips.  A
    supplemental form is sent home with your child prior to each specific trip, to be completed prior to the trip.

Dismissal Instruction Form
    This form is completed once per school year, providing instructions for school staff on where your child should go
    if school is dismissed early.

Employee/Volunteer Driver Information Sheet
    This form is completed if you will be transporting students to or from a school function (i.e. field trips, mile run)
    and is reviewed and updated at least once per year.

Free/Reduced Lunch Form
    Use this form to apply for free or reduced student lunches through the National Hot Lunch Program.  All
    information is kept confidential.

Home & School Cash Receipts Form
    Use this form if you are the chairperson of a fundraising event (or other event that collects money) to submit the
    money to the parish business office.  

Internet Use Agreement
    This form is completed by each student at the beginning of the school year, indicating understanding of and
    compliance with our internet policy.

Non-Prescription Medication Form
    This form is completed when any non-prescription (over-the-counter) medications, including cough drops, are to
    be taken by your child at school.

Parent Handbook
    This is the complete parent handbook, including school policies and procedures.

Prescription Medication Form
    This form is completed when any prescription medications are to be taken by your child at school.

Registration Packet (2011-2012)
    This document contains the forms for registering new students and continuing students each year.  It includes
    the Registration form and the Authorization for Automatic Payment form, as well as information on tuition policies
    and payments.

Scrip Order Form (Scrip fundraising program)
    Use this to order Scrip cards/certificates we keep on hand for about 50 of the most popular stores and
    restaurants.

Scrip Special Order Form (Scrip fundraising program)
    Use this to request Scrip cards/certificates for about 100 additional stores and restaurants.  These orders usually
    arrive 5-7 days after ordering.

Statement of Compliance
    This form is completed at the beginning of the school year, indicating compliance with our Parent & Student
    Handbook.

Tuition Scholarship Application
    This form should be completed by families seeking tuition assistance through the Diocese scholarship
    endowment trust.

Uniform Order Forms (click on specific item below)
    Use these forms to order monogrammed uniform pieces.  The school will place an order on the 15th of each
    month.


Volunteer Information Forms
    These forms are completed once by parents or others wishing to volunteer at the school. They are updated and
    initialed each year. The file contains 5 pages -- please be sure to print and complete all 5 pages.
